Waiver and Liability Form

1. Assumption of Risk
I acknowledge and understand that participation in physical exercise and fitness activities, including but not limited to the use
of gym equipment, participation in training programs, classes, or other services, involves inherent risks of injury, illness, or even
death. I voluntarily assume full responsibility for all such risks, known and unknown, and any resulting injury, illness, or damage.
2. Waiver and Release
I hereby release, waive, discharge, and hold harmless Hadeed Gym, its owners, managers, employees, independent
contractors, agents, successors, and assigns (collectively, “Releasees”) from any and all liability, claims, demands, actions, or
causes of action whatsoever arising out of or related to any injury, illness, damage, or loss that may be sustained by me while
on the premises or using the services and facilities of Hadeed Gym, whether caused by the negligence of the Releasees or
otherwise.
3. Indemnification
I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or costs, including court costs and
attorneys' fees, that may be incurred due to my participation in any activity, use of equipment, or presence on the premises.
4. Health Declaration
I affirm that I am in good physical condition and do not suffer from any condition that would prevent or limit my participation in
physical activities at Hadeed Gym. I understand it is my responsibility to consult a physician before beginning any exercise
program.
5. Rules and Guidelines
I agree to comply with all rules, policies, and guidelines established by Hadeed Gym. I understand that failure to adhere to
these policies may result in the immediate suspension or termination of my membership at the sole discretion of Hadeed Gym,
without refund.
6. Right to Refuse Service
Hadeed Gym reserves the right to deny or terminate membership or access to its facilities and services to any individual at any
time for any reason, including but not limited to failure to comply with gym policies, inappropriate conduct, or behavior deemed
harmful or unsafe to others.
7. Acknowledgment of Understanding
I have read this Waiver and Release of Liability Agreement fully and understand its terms. I acknowledge that I am signing this
Agreement freely and voluntarily and intend by my signature to release Hadeed Gym from any and all liability.
8. Minors (Under 18 Years of Age)
If the individual using the gym facilities is under the age of 18, a parent or legal guardian must sign below. By signing, the
parent or guardian acknowledges and agrees to all terms of this Agreement on behalf of the minor and assumes full
responsibility for the minor’s participation and conduct.
